<p>Care Manager RN – Maternal Health</p><p><strong>Location:</strong> Remote – Florida</p><p><strong>Duration:</strong> 3 Months Contract with possible extension or conversion</p><p><strong>Schedule:</strong> Monday–Friday, 8:00 AM–5:00 PM EST</p><p><br></p><p>Position Overview</p><p>We are seeking an experienced <strong>Registered Nurse Care Manager</strong> to support maternal-child health and high-risk Medicaid populations. This is a <strong>remote care-management position</strong> focused on helping members navigate complex healthcare needs throughout pregnancy, postpartum, and early childhood.</p><p>The Care Manager RN will serve as the clinical lead for assigned members, providing comprehensive assessments, developing individualized care plans, coordinating healthcare and community resources, conducting member outreach, addressing gaps in care, and supporting positive maternal and infant health outcomes.</p><p>Key Responsibilities</p><ul><li>Review referrals and risk-stratification reports to identify members requiring care-management support.</li><li>Conduct comprehensive and trimester-based assessments for pregnant and high-risk members.</li><li>Develop, implement, and regularly update individualized care plans.</li><li>Manage an assigned caseload and prioritize members based on clinical and social needs.</li><li>Conduct telephonic/member outreach to assess needs, provide education, encourage engagement, and coordinate care.</li><li>Coordinate services with physicians, OB/GYNs, hospitals, behavioral-health providers, community organizations, Healthy Start programs, and internal interdisciplinary teams.</li><li>Identify and address <strong>Social Determinants of Health (SDOH)</strong>, including transportation, food, housing, access-to-care, and other barriers.</li><li>Identify gaps in care and facilitate appropriate referrals and interventions.</li><li>Support <strong>Transition of Care (TOC)</strong> activities, including post-discharge follow-up and coordination.</li><li>Support members throughout pregnancy, postpartum, and First Year of Life programs.</li><li>Assist with behavioral-health integration and other maternal-child population-health initiatives.</li><li>Maintain accurate and timely documentation of assessments, interventions, care plans, and member interactions.</li><li>Work independently in a remote environment while managing multiple priorities and competing caseload demands.</li><li>Support case closure or graduation when established care-management goals have been achieved.</li><li>Meet established quality, productivity, member-engagement, regulatory, and documentation standards.</li></ul><p>Required Qualifications</p><ul><li><strong>Active, unrestricted Registered Nurse (RN) license.</strong></li><li>Minimum <strong>3 years of recent RN clinical experience</strong>.</li><li><strong>5+ years of nursing experience</strong> with one or more of the following:</li><li>Care Management</li><li>Case Management</li><li>Maternal-Child Health</li><li>Obstetrics / OB</li><li>High-Risk Pregnancy</li><li>NICU</li><li>Pediatrics</li><li>Population Health</li><li>Medicaid</li><li>Managed Care</li><li>Community Health</li><li>Experience managing a caseload and coordinating care across multiple providers, services, and community resources.</li><li>Strong clinical assessment, critical-thinking, and problem-solving skills.</li><li>Strong communication and member-engagement skills.</li><li>Ability to work independently in a fully remote environment.</li><li>Strong organizational, documentation, and time-management skills.</li><li>Ability to navigate multiple computer systems and applications simultaneously.</li></ul><p><br></p><p>Preferred Experience</p><ul><li>RN Case Management or RN Care Management experience.</li><li>High-risk OB or maternal-health care management.</li><li>Medicaid or managed-care experience.</li><li>Telephonic member outreach and engagement.</li><li>Experience addressing SDOH and coordinating community resources.</li><li>Experience with Transition of Care/post-discharge follow-up.</li><li>Behavioral-health integration experience.</li><li>Population-health or health-plan experience.</li><li>Experience working with interdisciplinary care teams.</li><li>Knowledge of maternal, postpartum, and infant-care programs.</li></ul><p><br></p>
