Location Requirement

  • Candidates may reside anywhere on the East Coast (EST time zone).
  • Must hold an active, unrestricted New York RN license.

Key Responsibilities

  • Conduct telephonic assessments using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS).
  • Perform in-person assessments for non-MLTSS members to evaluate medical needs and determine appropriate service referrals, including:
  • Adult Medical Day Care
  • Pediatric Medical Day Care
  • Personal Care Assistant (PCA) Services
  • Nursing Facility Custodial Care
  • Personal Preference Program (PPP)
  • MLTSS Enrollment
  • Work with diverse populations, including pediatric and medically complex members.
  • Coordinate care with members, authorized representatives, primary care providers, and interdisciplinary care teams.
  • Schedule and participate in interdisciplinary care meetings while advocating for members to ensure safe transitions and appropriate services.
  • Develop individualized care plans and authorize services within MLTSS/FIDE program benefits in a cost-effective manner.
  • Accurately document assessments, care plans, and member interactions in the electronic health record.
  • Utilize strong critical thinking and problem-solving skills in daily case management activities.
  • Mentor new staff after achieving role proficiency.

Required Qualifications

  • Active, unrestricted Registered Nurse (RN) license in the state of New York.
  • Minimum 3 years of clinical RN experience in one or more of the following:
  • Hospital
  • Home Health
  • Ambulatory Care
  • Fluent in English and Spanish (reading, writing, and speaking).
  • Associate Degree in Nursing (minimum).

Preferred Qualifications

  • Case Management experience
  • Discharge Planning experience
  • Managed Care experience
  • Crisis Intervention experience
  • Certified Case Manager (CCM) certification preferred

Position OverviewThe Nurse Case Manager utilizes a collaborative process of assessment, planning, facilitation, coordination, and advocacy to support members' healthcare needs. This role applies clinical judgment, evidence-based guidelines, and case management principles to promote quality, cost-effective outcomes while ensuring compliance with regulatory and accreditation standards.

Pay: $40.00 - $44.00 per hour
